Home-Based case managers work in the home and community with both children and adults to address mental health issues such as behavior difficulties, developmental delays, trauma and/or related concerns.

Targeted case managers organize behavioral or mental health services  with community resources to create a team approach 

Case managers from community support provide programs to help people acquire emotional regulation skills, crisis coping skills, social and interpersonal skills, medication adherence, and how to use community resources. 

These services, which are given in conjunction with home-based case management services, are intended to provide the best possible care.
