BEHAVIORAL HEALTH

Targeted Case Management

This service will be offered by a mental health, drug addiction and/or family specialist that are essential in helping the client achieve their rehabilitation and recovery objectives. 


Services provided will assist people in obtaining critical treatments, as well as reducing mental and addiction symptoms and developing optimum community functioning abilities. 


People in crisis will get aid and support, as well as service coordination, psycho education and support for individuals and their families, independent living skills, symptom monitoring and management skills, medication monitoring, and self-medication. 

On behalf of the beneficiary,  the case management providers will act as an advocate, broker, coordinator, and monitor of the service delivery system. 

Assessment

Case Managers conduct face-to-face assessments of a recipient's mental health, developmental disability issues, resulting in the issuance of an assessment report that includes a recommendation on whether the recipient meets target population criteria  to receive services.  This assessment serves as the foundation for the creation of a first Person Centered Plan.

Service Steps

  1. Defuse the current crises, evaluate its nature, and intervene to reduce the likelihood of a recurrence.
  2. Ensure linkage to needed community services and resources.
  3. Provide self-help and living skills training for youth.
  4. Provide parenting skills training to help the family build skills for coping with the clients disorder.
  5. Monitor and manage the presenting psychiatric and/or addiction symptoms
  6. Work with the clients  in the implementation of home-based behavioral supports. Services may include crisis management, targeted case management, individual and/or family therapy, substance abuse intervention, skills training, and other rehabilitative supports to prevent the need for an out-of-home, more restrictive services.
  7. This intervention uses an approach designed to address the identified needs of clients who are transitioning and are or maybe at risk of an out-of-home placement and need intensive interventions to remain stable in the community. This population has access to a variety of interventions 24 hours a day, 7 days a week, by staff that will maintain in contact and will  intervene as needed.


Service are individually designed for each client in full partnership with the family if they choose , to minimize intrusion, and maximize independence. Services are generally more intensive at the beginning of treatment and decrease over time as the clients as well as the families coping skills develop. The targeted case management services are structured and delivered face-to-face to provide support and guidance in all areas

In-Home Services

A interpersonal, relational intervention directed towards increasing an individual’s sense of well-being and reducing subjective discomforting experience. It is therapeutically supportive in nature.

Behavior Modification

To provide the client with redirection and modeling of appropriate behaviors in order to enhance his or her functioning within their home or community. The client’s plan of care should determine the focus of this service 

Family Support

To enable the family or caregiver (parent, guardian, custodian or persons serving in a caregiver role) to serve as a knowledgeable member of the client’s treatment team and to develop and/or improve the ability of families or caregivers to appropriately care for the client.

Crisis Management

A interpersonal, relational intervention directed towards increasing an individual’s sense of well-being and reducing subjective discomforting experience. It may be psychotherapeutic and/or therapeutically supportive in nature. Psychosocial Rehabilitation Behavior Modification

Home or Office Based

Our services are designed to produce behavior change. The client will need to have an active treatment plan and the goals must match the overall  services provided for the individual beneficiary. Clients will have to be part of participating in the stated common goals.

The Westbury Healthcare Solutions case managers offer a number of services for children and adults.

WHS's objective is to provide adapted living skills,  aided employment services, education, and/or other necessary resources to persons who are disadvantaged, imprisoned, or institutionalized who need case management services.
